Introduction:
Marijuana, also known as cannabis, has been a topic of debate and discussion worldwide. In India, where cultural norms and legal frameworks intersect, understanding the highs and lows of marijuana addiction becomes crucial. Let’s delve into the statistics and explore the impact of cannabis use in the country.
The Magnitude of Cannabis Use in India
According to the National Survey on Extent and Pattern of Substance Use in India conducted by the Ministry through the National Drug Dependence Treatment Center (NDDTC) at AIIMS in 2018, here are some eye-opening figures:
Prevalence
Among children and adolescents (aged 10-17 years), 0.9% are current users of cannabis.
Among adults (aged 18-75 years), 3.3% are currently using cannabis.
This translates to an estimated 2.9 million adults and 200,000 children using cannabis in India.
State-wise Variation
Cannabis consumption is higher in states like Uttar Pradesh, Punjab, Sikkim, Chhattisgarh, and Delhi compared to the national average.
Need for Intervention:
Approximately 72 lakh individuals in India require help for their cannabis use problems.

The Legal Conundrum
Despite growing calls for legalizing recreational cannabis use, most respondents in the India Today Group-Karvy Insights Mood of the Nation (MOTN) 2021 poll expressed opposition. 55% were against legalizing cannabis, while only 36% favored it.
The Global Context
Globally, cannabis is one of the most widely used psychoactive substances. India’s recent vote at the United Nations to remove cannabis and cannabis resin from the list of most dangerous substances indicates a shift in perspective. However, the debate continues.
Addressing Drug Addiction
Marijuana contains over 100 different cannabinoids, with the two most well-known and studied being THC (tetrahydrocannabinol) and CBD (cannabidiol). THC is the psychoactive compound responsible for the euphoric "high" associated with marijuana use, while CBD is non-psychoactive and is being explored for its potential therapeutic benefits.
THC's Impact on the Brain:
When THC enters the bloodstream, it quickly crosses the blood-brain barrier and binds to cannabinoid receptors in the brain. These receptors are part of the endocannabinoid system, which plays a role in regulating various physiological processes, including mood, appetite, and sleep. The activation of these receptors by THC leads to the release of dopamine, the "feel-good" neurotransmitter, creating the characteristic euphoria associated with marijuana use.
The Cycle of Marijuana Addiction:
While not everyone who uses marijuana becomes addicted, some individuals may develop a dependency on it. Regular and prolonged use of marijuana can lead to tolerance, where higher doses are needed to achieve the desired effects. This cycle of escalating use can pave the way for dependence and addiction.
Psychological and Physical Effects:
Marijuana addiction poses significant health risks encompassing both physical and mental well-being. Chronic use may lead to respiratory issues akin to tobacco smoking, causing chronic bronchitis and lung infections. Cognitive impairment, including short-term memory loss and reduced cognitive function, is prevalent, with potential permanent deficits in adolescents. Psychiatric disorders such as anxiety, panic attacks, and psychosis may arise, while depression can be exacerbated in susceptible individuals.
Addiction is a real concern, with tolerance development and withdrawal symptoms like irritability and insomnia upon cessation. Cardiovascular effects, including an increased heart rate and potential heart attack risk, are noted. Impaired driving skills and negative impacts on mental health, such as an increased risk of suicidal thoughts, should be considered. Social and legal consequences, including regulatory implications and social stigma, accompany marijuana use. Adolescents face vulnerability, with early use impacting brain development and educational outcomes.
Quality control issues, particularly in countries where marijuana is illegal, pose additional health risks due to contaminants and adulterants.
